Role Overview
- Schedule: Monday to Friday 5 days a week
- Provide patient care in a skilled nursing facility by conducting rounds in their short-term units
- Conduct documentation at home with provided iPad laptop within 48 hours of seeing patient

- Perform assessments and therapy sessions
- Collaborate with facility staff to enhance patient care
- Ensure timely and accurate documentation
Requirements
- Hold a valid full license as either a licensed professional counselor or a licensed mental health counselor
- Experience in healthcare or long-term care is preferred but not required
