Registered Dietician Job Summary We are seeking a passionate and experienced Registered Dietician to join our team at Absolut Care of Gasport, a leading provider of long-term care services in Gasport, NY. As a Registered Dietician at our care community, you will play a vital role in promoting the health and well-being of our residents by developing and implementing personalized nutrition plans that cater to their unique needs and preferences. Responsibilities Conduct initial assessments and develop effective nutrition care plans for residents, taking into account their medical history, dietary restrictions, and preferences Provide ongoing nutrition counseling and education to residents, families, and healthcare team members Collaborate with our interdisciplinary care team to ensure comprehensive care delivery and optimal outcomes Develop and implement menus and meal planning strategies that meet the nutritional needs of our residents Conduct regular monitoring and evaluation of residents' nutrition status and make adjustments to care plans as necessary Participate in Quality Improvement Initiatives and contribute to the development of best practices in nutrition care Maintain accurate and up-to-date records of resident assessments, care plans, and nutrition interventions About the Role As a Registered Dietician at Absolut Care of Gasport, you will have the opportunity to make a positive impact on the lives of our residents through your expertise and compassion.
