Job Description\n American Traveler is seeking a travel Home Health Physical Therapist for a travel job in Grand Junction, Colorado.Job Description & Requirements\n \n Specialty: Physical Therapist\n Discipline: Therapy\n Start Date: ASAP\n Duration: 13 weeks\n 40 hours per week\n Shift: 8 hours, days\n Employment Type: Travel\n \n Assignment Overview\n \n Shift: Days, 5x8hrs\n Hours: 40 hrs/wk\n Start Date: Sep 8, 2026\n Length: 13 weeks\n Openings: 1\n Description\n American Traveler is seeking a Physical Therapist for a home health position providing in-home skilled therapy services to patients in Grand Junction.Details\n \n Home health setting providing in-home physical therapy services\n Monday through Friday schedule, 8 am to 5 pm\n Guaranteed 40 hours per week\n Serves patients in the Grand Junction community\n One-on-one, patient-centered visits\n Uses Homecare Homebase (HCHB) for documentation\n Requirements\n \n Current, unrestricted Colorado PT license required (license required if applicable)\n Graduation from an accredited Physical Therapy program\n Valid driver's license and reliable transportation required\n Ability to practice independently in a home-based setting\n Proficiency or willingness to learn HCHB\n No minimum years of experience required\n Home health experience preferred\n Familiarity with Medicare documentation and home health regulations preferred\n Experience with geriatric and medically complex populations preferred\n Additional Information\n \n Perform physical therapy visits including evaluations, Start of Care, reassessments, recertifications, and discharge visits\n Develop and implement individualized plans of care based on patient needs and physician orders\n Provide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall prevention\n Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies\n Communicate patient progress and condition changes to the interdisciplinary care team\n May provide clinical oversight of Physical Therapist Assistants as applicable and permitted under Colorado law and agency policy\n Collaborate with RNs, PTAs, OTs, STs, and agency leadership on care coordination\n Ensure documentation complies with Medicare Conditions of Participation and HIPAA requirements\n Mission-driven home health services with supportive clinical and administrative coordination\n \n \n American Traveler Job ID #P-745180.
