Care Coordinator

Role Overview: The Care Coordinator supports children with developmental, physical, or emotional disabilities through assessment, service planning, coordination, and ongoing support. Key Responsibilities: Conduct functional assessments, develop individualized service plans, authorize and coordinate services, facilitate family-centered meetings, maintain accurate documentation, collaborate with teams, and participate in staff training. Service Delivery: Promote client independence, provide trauma-informed, family-centered services, utilize evidence-based practices, and ensure timely completion of documentation. Qualifications & Skills: Bachelor’s degree in a health or human services field, at least 1 year of experience working with children in the target population, strong communication and organizational skills, proficiency with electronic health records, and a valid driver’s license. Bilingual fluency in Spanish is preferred. Key Qualities: Compassionate, organized, able to respond to crisis situations, and capable of community-based travel. #J-18808-Ljbffr Rock Health
