About the Position

TravelNurseSource is working with Conflux Systems Inc to find a qualified Hospice RN in Barre, Vermont, 05641!

• Assess patient health status, pain levels, and symptom management needs
• Develop and implement individualized hospice care plans in collaboration with physicians and interdisciplinary teams
• Provide skilled nursing care, medication administration, wound care, and patient education
• Monitor patient condition and adjust care plans as needed to ensure comfort and quality of life
• Coordinate services with physicians, social workers, aides, chaplains, and therapists
• Educate patients and families regarding disease progression, medications, and end-of-life care expectations
• Maintain accurate and timely clinical documentation in electronic medical records (EMR)
• Respond to patient and family concerns with empathy, professionalism, and emotional support
• Participate in on-call rotations and provide emergency nursing support when necessary
• Ensure compliance with hospice regulations, safety standards, and company policies

Experience

• Experience with hospice case management required
• Experience providing hospice care in a home setting required. 
• Prior experience with rural travel preferred.
• Experience with McKesson/Netsmart EMR preferred
• Active RN License needed
• Acitve BLS/BCLS needed
