Case Manager

This position requires 13 weeks of 5x8 hour day shifts. Every fourth weekend and every other holiday are part of the schedule. The role emphasizes care coordination, discharge planning, and disease management. Candidates must have experience in needs assessment and plan of care development. First-time applicants must possess either acute care experience or EPIC experience. A background in case management is crucial for this role. Candidates must be able to provide documentation of all required certifications and references. Travelers may be required to float within their scope of practice as needed. Only candidates who meet the specified experience and certification requirements will be considered. A commitment to meet the standards of practice is necessary for success in this role.
