Case Manager

Role Description The Case Manager is a full-time, on-site role based in Iowa City, IA, working directly with young people and families to provide individualized support and advocacy. Day-to-day responsibilities include conducting intake assessments, developing service plans, and coordinating referrals to community resources such as housing, mental health, education, and employment services. The Case Manager maintains regular contact with youth and families through meetings, phone calls, and outreach, while tracking progress and adjusting plans as needs change. This role involves collaborating with internal program staff and external partners, facilitating communication between systems, and ensuring participants feel heard and respected. The Case Manager also completes documentation and reporting, participates in team meetings and trainings, and supports youth-focused programming as needed. Qualifications Ability to perform strengths-based case management, including assessment, goal setting, and service planning with youth and families. Experience building trusting, professional relationships with young people from diverse backgrounds and navigating crisis or high-stress situations with calm and empathy. Strong organizational and time-management skills to manage multiple cases, maintain accurate documentation, and meet reporting deadlines. Effective verbal and written communication skills for interacting with youth, families, colleagues, and community partners. Knowledge of community resources and social services systems (e.g., housing, education, mental health, juvenile justice) or willingness to learn quickly. Ability to work collaboratively in a team setting while also exercising sound judgment and independence in day-to-day decisions. Commitment to equity, inclusion, and culturally responsive practice when serving diverse youth and families. Proficiency with basic computer applications and case management or data entry systems. Bachelor’s degree in social work, psychology, human services, or a related field, or equivalent combination of education and relevant experience.
