Hospice Admission Nurse

Hartford Healthcare is seeking a Hospice Admission Nurse to coordinate and complete timely hospice admissions for patients and families. In this role, you will perform comprehensive nursing assessments, verify eligibility and obtain consents, develop initial plans of care, and collaborate closely with physicians, interdisciplinary team members, and community partners. You will educate patients and caregivers on hospice services, manage symptom relief at admission, and ensure seamless transitions of care.
