JOB REQUIREMENTS

Licenses: Active NY license
Board: Board Certified
Certifications: BLS, ACLS (AHA or RQI)
Experience: Minimum 2 years of experience
Other Qualifications: Epic experience required

DUTIES & RESPONSIBILITIES

Provide outpatient endocrine care, focusing on diabetes management.
Perform patient assessments, develop treatment plans, and follow up.
Manage insulin pump therapy and continuous glucose monitor data for diabetic patients.
Collaborate with a multidisciplinary team including 5 providers and 3 NPs.
Document patient encounters using EPIC electronic health record and AI ambient scribe.
