Key Responsibilities Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on resident goals and functional needs. Provide skilled interventions, including gait training, therapeutic exercise, balance training, manual therapy, and neuromuscular re‑education. Monitor resident progress and modify treatment plans as appropriate to maximize functional outcomes. Guide residents through their rehabilitation journey, from acute recovery through safe discharge and community reintegration. Collaborate with the interdisciplinary care team to coordinate effective, resident‑centered care. Supervise Physical Therapist Assistants (PTAs) in accordance with applicable California practice requirements. Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries. Participate in care conferences, clinical meetings, and quality improvement initiatives. Communicate effectively with residents, families, caregivers, and other members of the healthcare team. Graduate of an accredited Physical Therapy program. Strong clinical reasoning, communication, and interpersonal skills. Ability to work effectively within an interdisciplinary team.
