Recovery-Focused Mental Health Case Manager

Southwest Network is seeking a case manager for our adult outpatient clinics in Chandler, AZ. You will work with a clinical team to engage, educate and support adults living with serious mental illness, coordinating care among behavioral, medical and other providers. You will help members develop skills for independence, secure employment, and meaningful community involvement, with a focus on improved outcomes and quality of life.
