Position Summary: Responsible for establishing, implementing and evaluating a plan of care to restore function and prevent disability following disease, injury or loss of a body part.
Education: Masters Degree in Physical Therapy or Bachelor's degree in Physical Therapy with a minimum 5 years of work experience.
Licenses and Certification: Illinois license in Physical Therapy, BLS
Role Specific Responsibilities:
- Performs thorough evaluation
- Develops appropriate Plan of Care
- Implements Plan of Care
- Documentation is accurate, complete and timely
- Provides supervision to assistants, rehab techs, and students
Compensation (Commensurate with experience): $37.27 - $57.77
