Occupational TherapistHometown Home Health is seeking a licensed Occupational Therapist to provide in-home therapy services to patients throughout the Jacksonville area. This role is ideal for a clinician who enjoys working independently, building strong patient relationships, and helping individuals regain the skills and confidence needed to function safely in their daily lives.Hometown Home Health is a full-service home health agency serving patients throughout Greater Jacksonville and St. Augustine. Our clinical team provides skilled nursing, therapy, rehabilitation, and other medical services directly in the home, with care plans tailored to each patient's specific needs and goals.What You'll Be Responsible ForComplete occupational therapy evaluations and reassessments in patients' homesDevelop individualized treatment plans based on functional needs and physician ordersProvide therapeutic interventions focused on mobility, self-care, safety, and activities of daily livingEducate patients and caregivers on exercises, adaptive strategies, and safe techniquesCollaborate with physicians and other members of the home health care teamMonitor patient progress and recommend changes to the plan of care as neededComplete visit documentation and progress notes accurately and on timeQualificationsGraduate of an accredited Occupational Therapy programCurrent Occupational Therapist license in the state of FloridaCurrent CPR certification; online-only certification is not acceptedAt least one year of relevant clinical or institutional experienceAOTA membership preferredReliable transportation and ability to travel to patient homesStrong communication, organization, and patient-care skillsWhy Join Hometown Home Health?Home health gives Occupational Therapists the opportunity to work one-on-one with patients in the environment where their progress matters most.
