CoreMed Plus is seeking an experienced and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing outpatient primary care team. We are committed to providing high-quality, patient-centered care in a supportive and collaborative clinical environment.

Qualifications

• Minimum of 3 years of FNP/PA experience required
• Active DEA License required
• Experience in primary care and chronic disease management
• Strong clinical judgment and patient communication skills

Responsibilities

• Conduct patient assessments and develop treatment plans
• Diagnose and manage acute and chronic medical conditions
• Prescribe medications, order diagnostic tests, and interpret results
• Provide preventive care, wellness counseling, and health screenings
• Maintain accurate and timely EMR documentation
• Collaborate closely with physicians and clinical staff
