Per Diem Pharmacist Mount Sinai Hospital Inpatient

Description
The Per Diem Pharmacist serves patients by preparing medications; giving pharmacological information to multidisciplinary health care team; monitoring patient drug therapies.
Responsibilities1. Dispensesmedications accurately and in a timely fashion.
2. Resolvesquestions regarding formulary and non-formulary medications following policiesand procedures.
3. Providesdosing and therapeutic recommendations that take patient age/kinetic based onliterature.
4. Performsclinical chart reviews on a scheduled basis.
5. PerformsPatient Care Unit inspections on a scheduled and timely basis.
6. Preparesparenteral medications using sterile aseptic technique.
7. Maintainsadequate medication supply in the Pharmacy area and informs supervisor ofimpending shortages
8. Intervenesand documents interventions to correct/ improve pharmacotherapy.
9. Reportsadverse drug events and medication errors occurring in area serviced.
10. Takes responsibility for theoperation of the assigned Pharmacy area and activities of all supportive personnel.
11. Maintains pharmacologicalknowledge by reading professional publications attending workshopsconferences and C.E. programs to maintain professional competency.
QualificationsEducation Requirements
Graduate from an accredited College of Pharmacy (ACPE).
Experience Requirements
1 year minimum as licensed Pharmacist
Licensing and Certification Requirements (if applicable)
Licensed Pharmacist

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $111.91 - $111.91 Hourly. Actual salaries depend on a variety of factors including experience education and operational need. The salary range or contractual rate listed does not include bonuses/incentive differential pay or other forms of compensation or benefits.
