WHAT YOU'LL DO: As a Case Manager at the Crisis Intervention Center , you'll play a vital role in guiding individuals through some of the most challenging moments of their lives. You'll work closely with a multidisciplinary team to ensure clients receive the care, resources, and support they need both during their stay and after discharge. Here's what your day-to-day will look like:
- Coordinate admissions, transfers, and discharge planning in collaboration with the Clinical Director and care team.
- Serve as a key liaison between clients, families, legal reps, and community service providers.
- Develop and implement person-centered treatment and discharge plans focused on recovery, stability, and long-term success.
- Identify appropriate services or placements following discharge and ensure timely, effective referrals (housing, outpatient care, substance use treatment, etc.).
- Advocate for clients' rights and ensure their voice is heard throughout their care journey.
- Maintain up-to-date and accurate documentation of all case coordination efforts in compliance with HIPAA and KDADS regulations.
- Work to remove common barriers to discharge such as lack of housing, transportation, or insurance.
- Support the development of client-centered tools like WRAP (Wellness Recovery Action Plans).
- Complete all required training in trauma-informed care, de-escalation, crisis response, and more.

- Experience counts! We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college.
- Must be at least 21 years old.
- Experience in mental health, crisis intervention, or substance abuse treatment preferred.
- Ability to handle high-pressure situations and respond to emergencies.
- Must pass KBI and KDADS background checks.
- Must pass drug test.
- Maintain valid driver's license and insurable driving record (no substance-related incidents within 5 years).
- Must meet all health screenings, TB testing, and physical clearance requirements.
