Responsibilities
- Plan and conduct segments of treatment programs to restore and improve physical, social and mental functions; establish individualized treatment plans
- Conduct comprehensive evaluations and utilize appropriate clinical techniques and tools
- Provide therapy interventions per approved protocols; document progress and adjust plans as needed
- Develop individualized discharge plans and ensure communication of plans to patients, families, and care teams
- Supervise assigned therapy assistants; maintain accurate, timely clinical documentation within established deadlines
- Collaborate as a co-leader in an interdisciplinary patient care approach

Requirements
- Doctor of Physical Therapy (DPT) degree; required if license obtained on or after January 2016, or foreign equivalent
- Physical Therapy licensure in all applicable states of practice (current license in the state of assignment)
- CPR certification or Basic Life Support (BLS or HS-BLS or RQIBLS) from an approved vendor per policy
- Valid Driver’s License; age 21 or older
- At least two years of recent experience in the field
- EPIC experience required
- Ability to work in a Short Term Acute Care Hospital setting
- Active license, certifications, and education per the state of assignment as outlined in Job Notes
