Position Summary Performance Home Health is seeking a compassionate, skilled, and motivated Physical Therapist Assistant (PTA) to join our growing home health team. This field-based position provides high-quality physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist. The ideal candidate is patient-focused, organized, and committed to helping individuals improve their mobility, strength, safety, functional independence, and overall quality of life. Provide Patient Care: Deliver physical therapy treatments and interventions in accordance with the established plan of care and under the supervision of the Physical Therapist. Implement Treatment Plans: Carry out individualized treatment plans focused on improving strength, mobility, balance, endurance, safety, and functional independence. Monitor Patient Progress: Evaluate patient response to treatment and promptly communicate changes in condition, progress, or concerns to the supervising Physical Therapist. Educate Patients & Caregivers: Educate patients and caregivers on therapeutic exercises, home programs, fall prevention, mobility techniques, and safety practices. Complete Clinical Documentation: Accurately and timely document patient visits, interventions, progress, and outcomes within the electronic medical record (EMR). Maintain Compliance: Ensure all documentation and clinical activities meet organizational, regulatory, and professional standards. Collaborate Across the Care Team: Partner with Physical Therapists and other interdisciplinary team members to support coordinated, patient-centered care. Promote Patient Safety: Follow infection control protocols, patient safety procedures, and established standards of professional practice. Provide Field-Based Care: Travel through assigned service areas to provide therapy services in patients' homes. Required Qualifications Graduate of an accredited Physical Therapist Assistant program. Current and unrestricted Physical Therapist Assistant license in the state of practice. Current CPR certification. Valid driver's license, current automobile insurance, and reliable transportation. Strong clinical judgment, communication, time-management, and organizational skills. Ability to work independently and effectively manage responsibilities in a field-based environment. Preferred Qualifications Minimum of 3 years of home health experience. Experience utilizing electronic medical record (EMR) systems. Demonstrated ability to efficiently manage a flexible patient schedule. Strong knowledge of fall prevention, patient safety, therapeutic exercise, and functional mobility interventions within the home setting.
