Dentists: Multiple dentists needed to take evals through their current/existing practice. Practice locations must be within 75 miles of the following locations:
- Prattville, AL
- Madison, AL
- Salem, AL
- Fountain, FL
- Boca Raton, FL
- Stockbridge, GA
- Tifton, GA
- Cape Coral, FL
- Naples, FL
- Lake Worth, FL
- Tallahassee, FL
- Thomasville, GA
All Specialties: The Individual provider must go thru background checks and extensive credentialing, apart from our internal creds. Other requirements for LQTC. Providers must be submitted with CV in MO/YR format + explanations for any gaps greater than 30 days.
- Completion of approximately 16–18 hours of online self-study and a 2-hour live orientation prior to seeing Veterans. (Paid)
- Additional training only if required for specialized Independent Medical Review (IMO) cases.
- A training incentive upon successful completion of the required modules within two weeks of credentialing.
- A one-time ADA/OSHA site assessment of their office, conducted at Leidos QTC's expense.
