Registered Nurse- Per Diem

The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit. Qualifications Current RN license in the state where the employee will be working Active BLS certification from the American Heart Association or Red Cross Minimum one (1) year nursing experience; hospice or hospital experience preferred Previous experience working with an EMR/EHR system Valid driver’s license and automobile insurance per company policy (for mobile driver duties) Reliable transportation to meet visit schedule Ability to use equipment with visual and auditory mechanisms Effective communication in English (verbal and written) Flexible schedule to meet the needs of Hospice House patients Ability to manage the emotional stress of caring for terminally ill patients and their families Physical access to patient rooms and other areas of the Hospice House/Hospital/ALF, including navigating stairs and narrow spaces, and tolerating heat and lack of air conditioning in any part of the service area Ability to perform essential functions and physical requirements such as lifting patients and/or equipment, bending, pushing/pulling, kneeling For employees working at PACE: certification of completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs Some locations may require cross‑training to hospice house or field positions of equal licensure/certification and scope when census calls for flexible staffing Completes clinical admissions for all company services, including explaining services, identifying admission objections, educating families, completing informed consents, and initiating the Plan of Care Act as the company representative at assigned facilities, facilitating referrals and maintaining communication with admissions and after‑hours staff Meet productivity expectations while arranging safe discharge of referred and eligible patients Develop strong relationships with case managers, physicians, and other facility staff; work with Professional Relations Representatives to provide education on available services Provide care for any GIP patients in a hospital, including daily visits, professional management, documentation, and efficient discharge Complete competency requirements for this position satisfactorily Responsibilities Provides and manages direct care to patients and families as part of the Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical, and biological components Coordinates the Plan of Care, ensuring an individualized plan reflects the patient’s evolving needs Educates patients, families, caregivers, and other health professionals about disease process, palliative interventions, caregiving, dying process, and safety practices Reports changes in patient condition to appropriate members of the IDG or other health professionals Participates with the IDG to evaluate hospice referrals/admissions for level of care appropriateness Attends patient deaths; prepares body for transport and oversees disposal of medications Provides appropriate bereavement interventions to family and caregiver in conjunction with psychosocial staff Prepares concise and pertinent oral and written reports to the IDG, respecting and encouraging input from all disciplines Communicates accurately and completely to physicians, staff members, patients, families, and supervisors; utilizes positive approaches when working with others Supervises patient care given by other nursing team members (e.g., LPNs, Hospice Aides, contract nurses) During emergencies (e.g., hurricanes), may be required to report to a designated location to ensure continuity of services, including early arrival, overnight stays, or extended shifts Performs other duties as assigned Physical Demands Must see, hear, talk, walk, and use hands to finger, handle, or feel Frequently required to stand, sit, reach, lift, and bend Occasionally required to pull, push, stoop/crouch, kneel, or climb stairs Compensation & Benefits Hourly rate: $47.00 Position requires consent to drug and/or alcohol testing after a conditional offer of employment, and on‑going compliance with the Drug‑Free Workplace Policy All employees performing services for Florida affiliates are submitted through the Florida Care Provider Background Screening Clearinghouse for eligibility verification after a conditional offer and ongoing eligibility checks EEO Statement Chapters Health System is an Equal Opportunity Employer.
