Patient Care Navigator & Advocate
Proactive MD
Proactive MD seeks a Patient Advocate (PaD) to ensure continuity of care between the health center, community services, and patients. You will engage employees and dependents before, during, and after visits to provide personalized, informative, and compassionate care, reporting to the Director of Patient Care Navigation. You will work closely with Client Relations, Clinical Operations, the Health Center Provider, and Client HR to improve access and outcomes.
