Responsibilities Provide medical care, palliative treatment and manage terminal illness in compliance with the state Nurse Practice Act under supervision of a collaborating physician. Complete history and physical examinations by evaluating patient medical condition and health history. Complete hospice face‑to‑face visit requirements prior to the third and subsequent hospice benefit periods to assess eligibility. Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification. Maintain accurate clinical notes. Communicate with physician and participate in interdisciplinary meetings to meet medical and emotional needs of patient, family, and caregivers. Ensure compliance with all state and federal legal and regulatory requirements. Perform additional responsibilities as assigned. Qualifications Graduate from an accredited School of Nursing and an accredited Nurse Practitioner program. Current DEA license (if applicable). Current nursing license in state of practice. Current CPR certification. Master’s degree with a minimum of one year NP experience. One year hospice or palliative care experience preferred. Accreditation from an approved certifying body for advanced practice nursing required by state of practice. Certification in a specialist area preferred (e.g., Hospice and Palliative Nursing (CHPN) or Geriatrics). Commitment to clinical and documentation excellence. #J-18808-Ljbffr
