Travel Pay: $20.01/hour

Travel Rate (based on 40 hours): Hourly $20.01, Stipend (Lodging $1,141.00 + M&I $490.00) = $1,631.00,

Total Comp :$2,431.40

Travel Eligibility: Candidates must live at least 50+ miles from the facility to qualify for Travel Pay.

Shift Details:

• Sunday - Thursday

• 10am - 7pm or 11am - 8pm (Schedule can be flexible based on patient's availability.)

RN Case Manager Duties & Responsibilities:

• The Hospice Nurse admits patients to service, assesses the patient s and family s physical, psychosocial, environmental, safety and developmental needs.

• They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment.

• This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program and conducts follow up visits as needed.

**Minimum Qualifications **

• Current, valid, and unrestrictedCA license/certification in good standing, as required for the position.

• Minimum of one (1) year Hospice experience as RN Case Manager

• Registered Nurse license,

• Valid BLS

• Graduate of an Accredited School of Nursing

• Additional certifications may be required

• Copy of current, completedPrimary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

• Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
