Physical Therapist Position

Seeking a Physical Therapist for a home health position in Berkeley/Oakland. The candidate will provide therapeutic services to patients in their homes, ensuring quality care and adherence to clinical protocols.

Responsibilities:

• Evaluate patient conditions and develop individualized treatment plans
• Deliver therapy sessions in patients homes according to established protocols
• Document patient progress and update medical records accurately
• Communicate effectively with patients, families, and healthcare team members
• Comply with travel and documentation requirements, including Oasis experience
• Maintain licensure and certifications as required by state and agency standards

Qualifications:

• Hold a CA license in hand or be pending licensure approval
• Have experience with Oasis documentation; home health experience preferred
• Demonstrate strong clinical assessment and treatment skills
• Be BLS/CPR certified and compliant with travel regulations
• Possess reliable transportation for mileage coverage between patients
• Be willing to consider candidates with no prior home health experience
