Primary Responsibilities:
Perform lab draws and medication management support
Set up and assist with medication organization
Serve as a liaison between clients and physicians
Complete patient assessments and accurate documentation
Supervise Personal Care Assistants (PCAs)
Support coordinated, patient-centered care plans
Ensure continuity of care and communication across providers
Qualifications
Active Registered Nurse (RN) or Practical Nurse (LPN) license required for the state of Minnesota
Must pass a Minnesota background study
Compassionate, patient-focused approach to care
