Diabetes Educator (RN) - Patient-Centered Education & Care

Atlantic Health System in Morristown and Rockaway seeks a CDCES to coordinate individualized patient and support system education, planning and implementation of care goals. You will collaborate with referring providers and the interdisciplinary team, plan nutritional programs, assess cultural and learning needs, deliver tailored nutrition education, and document patient progress.
