Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to each patient's needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs for an effective return to community activities. The Physical Therapist serves as an educator for patients, families, students, coworkers, and themselves while maintaining compliance with the organization's Mission Statement and Core Values.

Responsibilities

• Complete comprehensive evaluations of assigned patients, developing appropriate treatment plans and goals focused on facilitating progress towards discharge.
• Document medical records clearly, accurately, and timely, including goal writing and functional quality indicator assessments.
• Ensure care delivery under signed, valid physician orders and within insurance approval as necessary.
• Participate in student programs by providing a positive learning environment, supervision, and evaluation.
• Create and pursue an individualized professional development plan, seeking continuing education and professional growth opportunities.
• Adhere to state practice acts and maintain professional licensure in good standing, completing all continuing education requirements.
• Engage with organizational performance improvement initiatives to enhance the quality of services provided.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Active CPR certification.
• Active and applicable state licensure.

Schedule

This position requires a 5x8.0 shift schedule. Start date is expected in the 3rd or 4th week of October.
