Responsibilities
- Provide direct patient care to residents in a long-term care setting within scope of practice
- Administer prescribed medications and document administration accurately
- Monitor resident status, document observations, and report changes to the supervising registered nurse
- Assist with activities of daily living and mobility, supporting resident independence and dignity
- Perform wound care, dressing changes, and basic treatments as directed
- Collaborate with the interdisciplinary team to plan and adjust care plans; ensure safety and infection control adherence

Requirements
- Active Licensed Practical Nurse license in New Hampshire
- Graduation from an accredited practical nursing program
- Current CPR/BLS certification
- Prior long-term care or skilled nursing experience preferred
- Ability to work evenings (15:00–23:30) and maintain 40 hours per week
- Local candidates to the Milford area
