Location: Sacramento County

Scheduled: Monday-Friday with on call weekends rotation

As a Home Health RN, you will:

• Provide skilled, compassionate nursing care in accordance with the nursing process, physician orders, and the patient's individualized Plan of Care (POC).
• Assess and monitor patient condition, regularly re-evaluating needs.
• Administer medications, treatments, and specialized procedures within scope of practice, training, and demonstrated competency.
• Communicate changes in patient condition and care needs with physicians, supervisors, patients, and families/caregivers to promote continuity of care.
• Educate patients and families/caregivers on medications, treatment plans, disease management, safety, and self-care to support independence.
• Complete accurate, timely, and thorough clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.
• Promote a safe, efficient care environment while supporting activities of daily living, appropriately delegating/supervising staff, and maintaining professional knowledge through ongoing education.

Required Qualifications

• Is a graduate of an accredited school of professional nursing, BSN preferred.
• Currently licensed to practice as a RN in the state of practice.
• If IV or ET certified, is currently certified and recognized by the state(s) where currently practicing as an RN.
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Possess and maintains a valid driver's license.

This position is based on a pay per visit compensation model. Pay per visit ranges from $100 -180 visit commensurate with experience and depending on type of visit.
