Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking three (3) part-time Community Health Educators for the Rural Health Transformation Program. All positions are 20 hours per week.
The Rural Health Transformation (RHT) Program is a federal grant that empowers states to strengthen rural communities across America by improving healthcare access, quality, and outcomes by transforming the healthcare delivery ecosystem.
Primary Responsibilities:
- Under the direction of the Manager of Nutrition Education, the Community Health Educator is responsible for increasing healthy living behaviors by providing health education using evidence-based programs to members in the HUSKY Health Program at health fairs, community, and school events.
- Activity will include the dispensing and collecting of evidence-based pre/post surveys and referrals to Community Health Worker program for Social Drivers of Health (SDOH) and benefit education support.
- Travels state-wide, teaching culturally appropriate health and nutrition information in a classroom settings and at community health and wellness events.
- Does statewide research for workshop and exhibiting opportunities, attends public health webinars, meetings and completes other projects assigned.
- Cultivates and maintains awareness of community cultures and values.
- Works collaboratively with community engagement team in coordinating partnerships and opportunities with community organizations.
- Educates and provides resources to all ages for health conditions such as obesity, hypertension, heart disease, stroke, diabetes, etc.
- Facilitate Social Drivers of Health (SDOH) and benefit education referrals to the Community Health Worker Ambassador Program.
- Participate in health fairs, group workshops, meetings, webinars, health promotion, and outreach activities online and in the community.
- Administer and collect pre/post assessments during nutritional workshops and completes data entry of surveys within established timeframe.
- Travel across the entire state of Connecticut and agrees' to mileage reimbursement policy.
- Completes necessary tasks before and after events, including printing/scanning materials, picking up/dropping off supplies, and office work.
- Review and respond to emails as needed.
- Work a combination of weekdays and weekend mornings, afternoons, and into the evening.
- This is a telecommuting position that requires working from a HIPPA compliant home workspace, via employee-supplied high-speed internet connection on company-supplied computer workstation and telephone technology.
- Attendance in person for meetings, training, etc. is required.
- Performs other related duties as assigned.
