Hospice RN Case Manager: Compassionate Care

Gentiva

Gentiva Hospice is seeking a compassionate RN Case Manager to lead comprehensive in-home hospice care. You will assess patients, coordinate with an interdisciplinary team, educate families, and ensure high-quality symptom management in patients’ homes and alternative settings.

The role emphasizes clinical excellence, empathy, and regulatory compliance. A current SC RN license, CPR, and a valid driver’s license are required, with on-call responsibilities as applicable.
