Registered Nurse (RN) - Home Health

Home Health RN – Nursing
Schedule - Shift: 8-hour days only
Assignment Details - Patient load: 1:1 ratio; may visit up to 6 patients/day - 30 productivity points standard
Requirements - Minimum: 1 year skilled home health RN experience required - Oasis experience: Required - BCLS (Basic Cardiac Life Support) certification required - Licensure: OR & WA RN licensure required (pending licenses accepted) - Key clinical skills: wound care (including wound VAC), disease management, PICC lines, G-tubes, ostomy care/education, catheter care
Roles & Responsibilities - Conduct patient assessments and develop care plans - Administer medications and treatments as prescribed - Provide wound care, including management of complex wounds - Educate patients and families on disease management and care procedures - Ensure compliance with documentation and reporting requirements
Compliance / Notes - Orientation: ~40 hours - Staff expected to have personal transportation; system groups clients to minimize drive time - Confirm vaccination requirements with hiring manager
