The opportunity

We are seeking an experienced Physician Assistant or Nurse Practitioner to provide outstanding longitudinal care and help guide the Healthspan360 membership program under physician medical leadership. This role combines excellent outpatient APP practice with program ownership: synthesizing complex data, implementing personalized plans, coordinating a multidisciplinary team, and making sure recommendations turn into measurable action.

This is a shared-caseload, co-managed model, not an independent panel. The APP performs the initial intake for every new member, and most subsequent visits — particularly complex, cardiometabolic, and higher-acuity care — are seen jointly with the collaborating physician. Members are HealthspanMD's members, not divided into separate APP and physician panels: the physician and APP function as one care team around each person.

This is a fit for a clinically grounded APP who wants more time with patients, enjoys prevention and complex data, communicates exceptionally well, and is energized by improving how care is delivered. It is not a volume-clinic role, it is not an unsupervised ‘wellness sales’ role, and it is not a solo panel — you will be working directly alongside physician leadership on the majority of your members’ visits.

Success in this role is measured by patient outcomes. Every plan, visit, and follow-up exists to move a member's actual health data in the right direction — cardiometabolic markers, cognitive and functional-performance measures, body composition, and the other numbers that define whether someone is truly getting healthier. That standard, not visit volume or satisfaction scores alone, is how this role and this program are evaluated.

What you will do

• Perform comprehensive initial intake visits for new Healthspan360 members, and provide longitudinal follow-up care — including history, examination, assessment, diagnosis, orders, treatment, prescribing, monitoring, and documentation — within Arizona scope and HealthspanMD protocols.

• Partner with the collaborating physician on most member visits, especially complex and higher-acuity care, so plans reflect joint APP and physician judgment rather than running as two separate practices under one roof.

• Help members implement and update individualized Healthspan360 plans spanning cardiometabolic health, prevention, brain health, physical performance, energy, sleep, body composition, medications, and behavior change.

• Review and synthesize advanced laboratory testing, imaging, cardiovascular assessments, DEXA, VO2, cognitive data, and functional-performance results.

• Manage appropriate medications and physician-approved treatment pathways, with disciplined indications, safety monitoring, consent, and escalation.

• Triage and respond to results, refills, and member questions; support the practice’s one-business-day communication standard and same/next-day urgent access model.

• Coordinate closely with Drs. Lisa and Robert Hurst, the preventive-cardiology APP, physical therapy/performance, Healthspan Strategist, member-experience team, and outside specialists.

• Maintain clear action plans, close clinical loops, and ensure every assigned member knows the priority, owner, and next step — including a clear record of which visits were APP-only versus joint with the physician.

• Build and improve practical clinical workflows, templates, education, quality checks, and program measures.

• Recognize emergencies and complexity early and escalate appropriately; maintain excellent HIPAA-compliant documentation and communication.

Required qualifications

• Current PA-C certification through NCCPA or current NP national certification appropriate to adult outpatient care (such as FNP or AGPCNP).

• Active, unrestricted Arizona license and prescribing authority — or a credible ability to obtain them on an accelerated timeline before clinical practice.

• Graduate degree from an accredited PA or NP program.

• At least 3 years of post-training clinical experience in outpatient internal medicine, family medicine, preventive cardiology, endocrinology/metabolic medicine, geriatrics, obesity medicine, or a closely related field.

• Strong clinical reasoning, medication-management, documentation, care-coordination, and patient-education skills.

• Comfort with high-touch service while maintaining evidence, ethics, boundaries, and efficient follow-through.

• Comfort working in a co-managed, shared-caseload model — coordinating tightly with physician leadership rather than practicing as an independent, unsupervised panel.

• BLS certification; ability to obtain ACLS if requested. DEA registration required if the assigned scope includes controlled substances.

Preferred qualifications

• 5+ years of relevant outpatient experience and a track record of safely managing medically complex adults.

• Experience with advanced lipid and cardiometabolic risk, weight management, hypertension, diabetes/insulin resistance, hormone care, cognition, sleep, or performance medicine.

• Experience in concierge, membership, executive-health, preventive, or longitudinal-care models.

• Evidence of building a pathway, service line, dashboard, protocol, or major workflow — not merely participating in one.

• Arizona PA collaborative-practice certification based on 8,000 qualifying hours, when applicable, or extensive NP autonomous-practice experience.

• Members show measurable, data-backed improvement in their health — cardiometabolic, cognitive, and physical-performance metrics trending the right direction over time — because that outcome, not activity, is the true measure of success in this role.

• Members feel heard, understand the plan, and receive timely answers without clinical corners being cut, whether that visit is with the APP or the physician.

• Physicians can trust that routine care and follow-up are accurate, complete, documented, and escalated when needed.

• Healthspan360 plans stay active across medical, cardiovascular, brain, and physical-performance domains.

• The APP/physician partnership feels like one continuous relationship to the member, not two separate providers — with clean handoffs and no dropped threads between intake, joint visits, and follow-up.

• The team has cleaner handoffs, fewer open loops, and a repeatable operating system that scales safely.
