Job Summary


Our client is seeking a dedicated Care Team RN, a pivotal clinical leader within the primary care team. This role involves acting as the main connection between patients and healthcare providers, combining advanced clinical judgment with efficient care coordination. The ideal candidate is a Registered Nurse skilled in high-level triage, complex patient education, and managing the clinical competence of the medical support staff to ensure seamless, high-quality care delivery.


Key Responsibilities

• Utilize the Nursing Process for advanced clinical triage to assess and direct patients to the appropriate level of care, serving as the clinical "air traffic controller" for both scheduled visits and emergent walk-ins.
• Develop care plans for "rising risk" patients, addressing physical, psychosocial, and social determinants of health.
• Support the professional development of the clinical team by assessing and validating Medical Assistants' skills and competencies.
• Partner with a team of 3 APPs and 1 Physician to manage complex paperwork, specialty referrals, and standing order protocols.
• Provide expert education on complex disease processes and medication regimens to empower patients for self-management.

Qualifications

• Current Colorado RN License.
• Current CPR (BCLS) certification.
• Graduate of an accredited Nursing Program required; BSN preferred.

Experience

• Two years of experience in case management and clinical nursing preferred.

Skills

• Basic computer skills: Manage timecard, review policies, use emails, modify workbooks, and create presentations.
• Preferred experience with Epic and familiarity with Medicare/Medicaid paperwork.
• Bilingual Spanish skills preferred.
