We are seeking a dedicated and compassionate Physical Therapist to join a rehabilitation team. We offer separate Full-Time and Weekday PRN opportunities, providing flexibility while making a meaningful impact on resident care.

Available Positions

• Full-Time Physical Therapist

• PRN Physical Therapist – Weekdays Only

Position Overview

The Physical Therapist evaluates residents, develops individualized treatment plans, and delivers therapy services that promote functional independence and improved mobility. This role requires strong clinical judgment, a patient-centered approach, and collaboration with the interdisciplinary care team.

Key Responsibilities

• Provide physical therapy services in accordance with established standards of practice

• Conduct evaluations and develop individualized treatment plans with measurable goals

• Implement therapeutic interventions to improve strength, balance, and mobility

• Collaborate with nursing and therapy teams to ensure coordinated care

• Maintain accurate and timely clinical documentation

Qualifications

• Active Physical Therapist (PT) license in the applicable state

• Current BLS certification

• Skilled nursing or geriatric experience preferred
