Key Responsibilities\n

• Act as a liaison between families, outpatient providers, and our organization, ensuring clear and effective communication to meet client needs.

• Engage with insurance companies for pre-certification processes and utilization management, advocating for the best possible care for each client.

• Participate in discharge planning and aftercare strategies, working collaboratively with multidisciplinary teams to create comprehensive care plans.

• Provide therapeutic meal support, actively modeling and teaching healthy relationships with food to clients, as well as addressing any emotional aspects associated with nutrition.

• Deliver empathetic and affirming care tailored to each client's unique circumstances, paying close attention to their intersecting identities and experiences.

Compensation and Benefits\n
