Key Responsibilities\n- \n
- Act as a liaison between families, outpatient providers, and our organization, ensuring clear and effective communication to meet client needs. \n
- Engage with insurance companies for pre-certification processes and utilization management, advocating for the best possible care for each client. \n
- Participate in discharge planning and aftercare strategies, working collaboratively with multidisciplinary teams to create comprehensive care plans. \n
- Provide therapeutic meal support, actively modeling and teaching healthy relationships with food to clients, as well as addressing any emotional aspects associated with nutrition. \n
- Deliver empathetic and affirming care tailored to each client's unique circumstances, paying close attention to their intersecting identities and experiences. \n

Qualifications\n- \n
- A Master's Degree in Social Work from an accredited institution. \n
- A valid LCSW license in the state of New Jersey is required. \n
