Physical Therapist Assistant (PTA) | Home Care | Cornerstone VNA Location: Rochester NH 03867 Job Type: Full-Time or Part-Time Help Patients Move Better, Safer, and Stronger At Home Cornerstone VNA is seeking a skilled and compassionate Physical Therapist Assistant (PTA) to join our Home Care Rehabilitation team. In this role, youll work under the supervision of a licensed Physical Therapist to deliver medically prescribed therapy services in the home setting, helping patients restore function, improve mobility, and maintain independence. What Youll Do Patient Care & Treatment Provide physical therapy treatments in the home setting according to an established plan of care. Assist patients in restoring function, improving mobility, relieving pain, and preventing further physical decline. Utilize appropriate therapeutic techniques, exercises, and interventions within PTA scope of practice. Prepare for visits and organize daily schedules to maximize efficiency and minimize travel time. Documentation & Compliance Accurately document treatments, standardized tests, patient education, and functional progress in the EMR. Ensure documentation reflects visit-to-visit progress or interventions aimed at preventing deterioration. Synchronize documentation per agency standards and timelines. Care Coordination & Collaboration Work closely with the supervising Physical Therapist to support patient goals and outcomes. Communicate effectively with interdisciplinary care team members. Adhere to scheduling processes and assist with efficient care delivery. Utilize resources appropriately and support referrals to other disciplines when indicated. Professional Development & Quality Participate in quality improvement initiatives and performance self-evaluations. Complete required trainings, competencies, and in-services. Incorporate continuing education and specialty skills into clinical practice as appropriate. What You Bring Current Physical Therapist Assistant (PTA) license in New Hampshire and Maine Graduation from an approved PTA program 2+ years of therapy experience preferred Home care experience preferred (minimum 1 year) Strong clinical judgment and independent problem-solving skills Excellent communication, organization, and documentation skills Comfort working independently in a home-based environment Why Cornerstone VNA?
