Case Manager-Emergency Housing Voucher (EHV)

3rd Street Youth Center & Clinic is a community-based agency providing youth in the Bayview Hunters Point with medical and behavioral health services that encourage them to make decisions that support their health, safety, and development. 3rd Street Youth Center & Clinic also manages a number of successful Transitional Age Youth (TAY) housing stabilization programs and one of the city’s few Youth Access Points The goals of Emergency Housing Voucher program (EHV) are to reduce the length of time that tenants spend experiencing homelessness, and to ensure that tenants retain housing once established. The program will serve Transitional Age Youth (TAY ages 18 to 24) and those ages 25 to 27 who have been a part of the Homeless Response System (HRS). The Emergency Housing Voucher Case Manager (EHVCM) will provide all necessary services to ensure a seamless transition to permanent housing. The EHVCM will assist tenants with securing needed documentation to move into housing, provide housing navigation services, and working closely with EHV administrator(s) to ensure that all needed services are in place prior to housing placement. The EHVCM will provide wrap-around case management services within a harm reduction model to ensure tenants’ long-term housing retention and improved well‑being. Duties & Responsibilities: The following is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the EHVCM. Duties, responsibilities, and activities may change, or new ones may be assigned as needed. Engagement with all tenants referred for housing placement to determine preferred housing options, required services, and needed documentation; Housing Navigation services to assist successful transition into permanent housing, including unit viewings and selection, accompaniment during move- in process, and orientation to neighborhood and surrounding services; Arranging for necessary services after housing placement, such as In-Home Support Services (IHSS), or care by a medical or behavior health provider; Provision of targeted services and/or referrals to another appropriate agency for tenants whose behavior indicates a substance abuse, mental health or another issue that is jeopardizing the tenant’s housing retention and/or health; Linkages to community resources, case management, and crisis intervention within a Housing First, trauma-informed, and harm reduction modality; Education on tenancy requirements and support to address barriers to housing retention; Support with completing any required processes for housing provider’s income certification and re-certification processes; and Ongoing coordination with EHV partners who are serving the tenant, through meetings, calls, and other communication, as needed. About you: Skills, Knowledge, and Abilities (SKA): Excellent verbal and written communication skills Strong interpersonal skills Basic computer knowledge, MS Word and Excel required; PowerPoint a plus Knowledge of HMIS/One system preferred Flexibility required regarding scheduling and prioritizing of tasks Ability to utilize critical thinking skills in decision-making situations, good organizational and record-keeping skills, and good independent judgment Bilingual a bonus Education & Experience: Bachelor’s Degree in Social Work or a related field Experience with homeless, developmentally-disabled, and/or other marginalized populations Experience with Transition Age Youth (TAY) or Case Management experience Knowledge of different housing models, particularly section 8 and rapid rehousing Certificates, Licenses, and other Possession of a valid California driver’s license, a clean driving record, and automobile insurance a plus Access to reliable transportation Travel requirement This position requires frequent visits to properties, attendance to on-site vendor meetings, community meetings, funding workshops, trainings, and other events primarily in the San Francisco Bay Area. Opportunities to attend events outside San Francisco’s Bay area, including Brilliant Corners’ sponsored events throughout California, or nationally may also be presented for this position. Willingness to travel and make multiple visits in the field Availability: must be available to work weekends General info: This position is a salaried, exempt and full-time role. This position is in our Housing office located in the Bayview/Hunter’s Point district in San Francisco. This position reports to Director of Housing & Assistant Director of Housing. Compensation and benefits: This position has an annual salary of $70,304.
