Job Description

Hospice Registered Nurse (RN) – Home Health | Wisconsin

We're seeking a compassionate Registered Nurse (RN) to provide in-home hospice care throughout Fond du Lac, Dodge, and Columbia Counties , with additional opportunities in Jackson, Waterford, and Sheboygan .

Schedule

Monday–Friday, 8:00 AM–5:00 PM
No weekends or on-call
Approximately 5 patient visits per day
Mileage reimbursement: $0.53/mile

Responsibilities

Provide skilled hospice nursing care in patients' homes
Perform comprehensive assessments and develop care plans
Deliver wound care, pain management, and symptom management
Educate patients and families on end-of-life care
Coordinate care with physicians and the interdisciplinary hospice team
Complete accurate EMR documentation

Requirements

Active Wisconsin RN license
Current CPR/BLS certification
Wound care experience
Valid driver's license, reliable vehicle, and auto insurance

Preferred

Hospice experience
HomeCare HomeBase (HCHB) experience preferred (MatrixCare or other EMR experience accepted)
Case management or home health experience
