This opportunity is with a well-established hospice team in the North Suburbs of Chicago that is focused on creating a more sustainable and supportive environment for nurses doing this important work. What makes this role different A team that understands the emotional and clinical demands of hospice care Structured support, including dedicated after-hours nurses (you're not carrying everything alone) Thoughtful approach to scheduling and patient coordination The ability to work independently while still feeling supported The role As a Hospice RN Case Manager, you'll manage and coordinate care for patients in a home-based setting, working closely with an interdisciplinary team to ensure comfort, dignity, and quality of life. You’ll be responsible for: Managing patient care in hospice settings Coordinating with physicians and care teams Supporting patients and families through all stages of care Providing leadership and guidance within the care team Schedule & structure Monday–Friday schedule (daytime hours with flexibility in start/end times) Limited on-call expectations:

1. weekend day per month (scheduled visits + overnight on-call)
2. weekday on-call shifts per month

Dedicated after-hours RN team in place for additional support Additional compensation provided for on-call coverage 2 holidays per year + floating holidays Territory Local travel within a defined service area Focus on maintaining reasonable coverage and minimizing unnecessary travel What they're looking for 1+ year of hospice or home health experience Experience in case management or care coordination Strong clinical judgment and ability to work independently A passion for patient-centered, end-of-life care Certifications: AHA BLS (required) AHA ACLS (required) Compensation $70,000 - 90,000annually Additional stipends for on-call coverage Location Openings available in Elmhurst, McHenry, Mount Carmel,Mount Pleasant, Mount Vernon,Northbrook, & Wheeling,IL A note for experienced nurses If you've worked in hospice or home health, you know how important it is to have the right team, manageable expectations, and support in place.
