We are seeking an experienced Clinical Monitor to join our Independent Monitoring Unit. This is an excellent opportunity for a mission-driven clinical professional who is passionate about disability rights and wants to use their expertise to make a meaningful difference in the lives of people with developmental and intellectual disabilities and/or mental illness.

About the Role

As a Clinical Monitor, you will work as part of a collaborative team to monitor the quality and safety of services provided to people with disabilities across Illinois. You will conduct on-site monitoring visits at facilities and community-based programs, assess services and supports, and help ensure that individuals receive appropriate care in safe, respectful, and person-centered environments.

A key part of this role is supporting successful transitions from institutional settings into the community and identifying opportunities to improve services and outcomes. You will also analyze information gathered through monitoring activities, prepare written reports, and develop recommendations that can contribute to meaningful systemic change.

This role is ideal for someone who combines strong clinical judgment with excellent communication skills and a genuine commitment to protecting and advancing the civil rights of people with disabilities.

Key Responsibilities

• Conduct on-site monitoring visits at facilities and community programs throughout the Northern Illinois region.
• Assess the safety, quality, and appropriateness of services and supports provided to individuals with disabilities.
• Monitor individuals' progress toward successful community integration and transitions.
• Collaborate with other monitors and members of the Independent Monitoring Unit to identify concerns, trends, and opportunities for improvement.
• Review records, interview individuals and service providers, and gather information relevant to the quality and delivery of services.
• Analyze monitoring findings and prepare clear, thorough written reports.
• Develop recommendations designed to address individual concerns and promote systemic improvements.
• Communicate professionally and effectively with individuals with disabilities, families, providers, government agencies, and other stakeholders.
• Advocate for services and practices that respect the rights, dignity, independence, and individual needs of people with disabilities.
• Travel locally and overnight as needed to conduct monitoring activities.
