Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 08/20/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Title Physical Therapist - SNF Location Phoenix, AZ Schedule / Job Type Days | Contract (Open to Temp, Temp-to-Hire, or Direct Hire) Key Responsibilities Establish and administer individualized treatment plans with specific therapy goals based on patient capacity and tolerance, under physician direction. Assess patient needs and plan, evaluate, and modify care to meet physical therapy intervention goals. Collaborate with interdisciplinary team members to plan and evaluate team goals for each patient. Document and record patient and resident condition and progress in accordance with state and federal regulations. Educate patients and families on appropriate therapy methods and techniques. Communicate patient status and needs to patients, families, and other involved healthcare professionals. Required Qualifications Bachelor's, Master's, or Doctorate Degree from an accredited Physical Therapy program. Current Physical Therapist license in the state of employment. Current CPR certification. Preferred Experience / Skills Minimum of six months of experience in a long-term care or skilled nursing facility environment. Demonstrated ability to set realistic and achievable goals for patients and residents in a SNF setting.
