Job Overview: We are seeking compassionate and dedicated Home Health Professionals to assist individuals with daily living activities and provide essential care and companionship. This role involves supporting individuals in their homes by promoting hygiene, assisting with personal care, managing medications, and helping with household tasks. Key Responsibilities: Assist individuals with personal care needs, including bathing, showering, toileting, shaving, dental and nail care. Promote good hygiene and grooming habits. Support individuals with household chores and daily tasks. Provide companionship and emotional support. Accompany individuals to medical appointments and community activities. Implement and document interventions as outlined in the Individual Services Plan (ISP). Maintain and review documentation regarding individual progress and skill development. Run errands and assist with meal preparation. Work a flexible schedule based on client needs and assignments. Qualifications: High school diploma or equivalent (Required). At least 1 year of caregiving experience (Preferred). Valid Driver's License (Preferred). First Aid/CPR Certification (Preferred).
