Our Adult Medical Day Care is seeking a dedicated and compassionate Certified Social Worker to join an amazing program and professional healthcare team. Your energetic and professional approach will help foster a positive environment for participants, family and colleagues alike. Position is 8 AM to 4:00 PM with an hourly wage of $25 and a sign‑on bonus of $500. No weekends, evenings, or nights. Applicants with either Adult Medical Day Care or long‑term care experience is a plus. Responsibilities Interviewing the participant and caregiver to obtain a social assessment and evaluation of needs and problems. Assessing the government authority with completion of documents required by the new jersey adult daycare, such as an application etc. Quarterly verification of participants’ Medicaid eligibility. Medicaid, food stamps, application and renewal housing application etc. Care plan development quarterly.
