Title
Home Health Clinician - Skilled Home Visits

Schedule
Every 3rd weekend required. Visits/hours are self-scheduled based on office needs.

Assignment Details
- Visit load: 6-8 skilled visits/day.
- Orientation: 16 hours.

Requirements
- Minimum 2 years clinical experience.
- Must have home health experience.
- BLS certification is required.
- State license (FL or compact license) is required at the time of submittal.
- OASIS training preferred.
- Experience with cardiopulmonary care, geriatrics, neuro and ortho management, and oncology preferred.

Roles & Responsibilities
- Conduct patient assessments and develop home exercise programs.
- Monitor vital signs and manage pain for patients.
- Provide education to patients and families regarding safety and injury prevention.
- Utilize assistive devices and supplemental oxygen delivery as necessary.
- Administer medication and monitor for compliance.

Compliance / Notes
- COVID-19 vaccine required (medical/religious exemptions allowed).
- Flu vaccine proof or declination is required; masks are mandatory if declination is provided.
- Compliance documents are due two Fridays before the Monday start date.
- A pass/fail EKG exam is required (up to 3 attempts).
- Return-to-work policies apply; discuss with management if this is a concern.
