Part-Time Occupational Therapist

Innovive Health is seeking a compassionate and experienced Occupational Therapist to provide high-quality care to patients in their homes. In this role, you will evaluate patients, develop individualized treatment plans, and provide occupational therapy services that help patients improve their independence, safety, and ability to perform activities of daily living.

The Occupational Therapist will work collaboratively with physicians, nurses, rehabilitation professionals, patients, and families to deliver patient-centered care in accordance with physician orders, applicable regulations, and Innovive Health policies.

Part time 16 hours a week - you can choose two 8 hour weekdays or four 4 hour weekdays

What You’ll Do

• Conduct occupational therapy visits in patients’ homes, including evaluations, follow-up visits, 30-day re-evaluations, recertifications, and discharges.
• Evaluate patients’ rehabilitative needs and contribute to the development of individualized plans of care.
• Develop and implement occupational therapy treatment programs based on each patient’s needs and goals.
• Help patients improve their ability to safely participate in activities of daily living through therapeutic exercises, transfer training, neuromuscular re-education, coordination activities, and safety and fall-prevention education.
• Educate and support patients, families, and caregivers throughout the course of treatment.
• Communicate with physicians regarding therapy frequency, duration, treatment plans, progress, and goals.
• Assess vital signs, including blood pressure, heart rate, and oxygen saturation, as appropriate for patient safety, and report abnormal findings to the appropriate clinical team and physician.
• Educate patients on the proper use of durable medical equipment, including walkers, grab bars, shower chairs, wheelchairs, crutches, canes, and prostheses, and coordinate equipment needs when appropriate.
• Accurately document patient visits, progress, responses to treatment, and recommendations using the electronic medical record.
• Collaborate with nurses, occupational therapy assistants, physical therapists, physical therapist assistants, speech-language pathologists, physicians, and other members of the interdisciplinary care team.
• Participate in discharge planning and educate patients and caregivers on home exercise programs, assistive equipment, and recommendations for continued care following discharge.
• Maintain timely and accurate clinical documentation and progress summaries.
• Follow all agency policies, confidentiality requirements, infection-control practices, safety procedures, and applicable federal, state, and local regulations.
• Maintain professional and clinical knowledge through continuing education and professional development.
• Perform other duties as assigned.

Required Qualifications

• Graduate degree in Occupational Therapy from an accredited college or university.
• Current and valid Massachusetts Occupational Therapist license.
• Current CPR certification.
• At least one year of experience in home health, skilled nursing, acute care, or inpatient rehabilitation.
• Valid driver’s license.
• Reliable access to a vehicle.
• Active automobile insurance.

Preferred Qualifications

• At least one year of home health experience.

Physical Requirements

This position requires the ability to perform the essential functions of the role, including:

• Frequent standing, walking, and sitting.
• Frequent use of hands and fingers.
• Frequent bending, stooping, kneeling, balancing, climbing, and crawling.
• Ability to occasionally lift, push, or move more than 25 pounds.
• Ability to occasionally assist with moving, lifting, or transferring patients.
• Frequent exposure to bloodborne pathogens, airborne pathogens, and other infectious materials.

Travel Requirements

This is a field-based position requiring daily local travel by car to complete patient visits and attend in-person meetings, training, and continuing education. Occasional travel to corporate offices or locations outside the assigned service area may also be required.
