Behavioral Health Care Manager

Humana is seeking a Field Care Manager, Behavioral Health to coordinate and deliver comprehensive behavioral health care for members in the community. In this role, you will conduct in-home or virtual assessments, develop person-centered care plans, and collaborate with physicians, social workers, and community providers to address mental health and substance use needs. You will monitor progress, close care gaps, ensure appropriate utilization of services, and support care transitions. The role requires strong clinical judgment, documentation skills, familiarity with managed care, and a commitment to culturally competent, member-focused care and outcomes improvement.
