Position Summary We are seeking a licensed Physical Therapist (PT) with home health experience to join our growing team. This role involves delivering skilled physical therapy services to patients in their homes, collaborating with an interdisciplinary team, and supporting patients in achieving their rehabilitation goals. Key Responsibilities Provide skilled physical therapy services in a home health setting Perform patient evaluations and develop individualized treatment plans Implement evidence-based interventions to improve mobility and function Educate patients and caregivers on home exercise programs and safety Document patient care accurately using EMR Collaborate with nurses, physicians, and care team members Ensure compliance with all state, federal, and agency regulations Qualifications Active Physical Therapist (PT) license in Indiana (required) Minimum 1 year of home health experience (required) Valid driver’s license and reliable transportation Strong communication, time management, and organizational skills Compensation & Benefits Competitive salary Health insurance Dental insurance Paid time off (PTO) Paid holidays Why Join Us?
