The Child Center of NY
The Child Center of NY in New York City seeks a Permanency - Family Connections Specialist, RTF to promote timely discharge planning and lasting family ties for youth in our Residential Treatment Facility. You will use the Family Finding model to engage families and supportive adults, coordinating with care teams, counties, and partners to build lasting networks. The role includes supervision of the Family Peer Advocate, collaboration with Transitional Coordinators, and expanding resources to #J-18808-Ljbffr The Child Center of NY
