The Division of Disease Control's mission is to safeguard the health of New Yorkers through identification, surveillance, treatment, control, and prevention of infectious diseases, which is achieved through varied and interrelated endeavors of its seven Bureaus. The Bureau of Public Health Clinics (BPHC) mission is to promote a healthy community by providing New Yorkers with the resources needed to make informed and empowered health decisions; identify and treat tuberculosis and provide immunization and sexual health services regardless of ability to pay or immigration status. BPHC provides direct clinic services to people seeking health care; monitors disease trends; provides education and training to service providers and community groups; conducts research; and develops policies and programs to deliver high‑quality clinical care which best serves New Yorkers. The Bureau of Public Health Clinics seeks to hire a part‑time Physician. Duties Will Include But Not Be Limited To Performs direct patient care in his/her clinic 5 days a week. Manages day‑to‑day clinical care of patients within his/her chest center. Supervises other physicians in the clinic and completes performance evaluations in a timely manner. Coordinates review of labs and chest x‑rays and coordinates the follow‑up of abnormal results with the patient in timely fashion. Ensures that all TB disease or suspected TB patients (Class III & V) patients in the Center have been reported and that every effort is made to ensure follow up & complete appropriate treatment. Ensures that Directly Observed therapy (DOT) is given to as many patients as possible. Reviews the records of patients on clinic and community‑based DOT. Recommends appropriate cases for intermittent DOT. Performs QA on medical records of other physicians to ensure that prompt, effective treatment is given to priority patients (TB disease, Suspected TB disease, contacts, immunosuppressed or HIV patients). All contacts, HIV‑infected persons on treatment for latent TB infection should be monitored intensively by the PIC. Meets weekly with PHAs and other clinic staff to conducts case management on all Class III patients in the Center, ensuring that appropriate treatment is being given, that contact is made within 24 hours of a missed clinic visit, that an appropriate contact investigation and monitoring is occurring, that the patient has been brought back to treatment if needed, etc. Follows up with Case Manager and appropriate staff on these issues. Drug resistant/drug intolerant TB patients, including MDR and rifampin resistant, are reviewed monthly at a minimum and when needed. Ensures that case management is documented in the TB registry system (maven). Communicates and interacts with non‑DOH medical providers and health care workers as needed to reinforce guidelines for high standard TB treatment. Performs 2 month and end of treatment reviews of all confirmed cases in the chest center. Regularly reviews medical records of other clinicians working at the clinic. Any problems need to be documented, and appropriate action taken. Works with center's administrative managers to ensure that medical records at site are maintained, correct forms are being used, and proper documentation is done. Deals with emergency situations in the Center. Assists the clinic manager in the coordination of flow of patients throughout the clinic. Review and coordinates implementation of new TB control policies and protocols, working closely with nurse managers, other PICs, medical director of BPHC and BTBC. Performs Suspect Review by reviewing the medical record and/or discussing the cases with the case managers and nursing staff at the clinic. Ensures that all suspect cases are dispositioned in the appropriate time frame. Gives input to the Directors of Medical Affairs of BPHC and BTBC about upcoming and current TB bureau policies. Assists in the orientation of new staff as needed. City Medical Specialist (PART - 53040) Minimum Qualifications Possession of a valid license to practice medicine in the State of New York; and either: Valid Board Certification issued by the appropriate American Specialty Board in any specialty area required by the agency; or Current approved application on file for admission to the certifying examination given by the appropriate American Specialty Board in any specialty area required by the agency. To be assigned to Assignment Level II, individuals must have, in addition to meeting the minimum qualification requirements for Assignment Level I described above, one year of satisfactory experience practicing in the specialty area. To be assigned to Assignment Level III, individuals must possess: A valid license to practice medicine in the State of New York; and A Master's degree in Public Health, or equivalent Master's degree, including or supplemented by graduate credits in epidemiology and biostatistics. Completion of Epidemic Intelligence Service program of the Center for Disease Control and Prevention may be used to substitute for this Master's degree; and At least two years of satisfactory experience after receipt of the Master's degree, or equivalent, in the practice of epidemiology. Specialty Board Certification or eligibility may be substituted for one year of this experience. To be assigned to Assignment Level IV, individuals must possess: A valid license to practice medicine in the State of New York; and Completion of an approved residency program in an accredited hospital, including three years of experience, in either internal medicine, or family practice, or pediatrics, or another related specialty area required by the Department of Health and Mental Hygiene; and Valid board certification or board eligibility issued by the appropriate American Subspecialty Board in Infectious Disease, Pulmonary Medicine, or a related subspecialty; and Completion of an approved fellowship program in an accredited hospital, including two years of experience, in either infectious diseases, or pulmonary medicine, or another related subspecialty required by the Department of Health and Mental Hygiene.
